Auction/Dessert Item Donation Form

Please fill out the form below if you are donating an item for our silent or live auction OR a dessert for our Dessert Frenzy.

Auction Items: in the item description please specify item size, color, number or persons, time limitations and any other restrictions or information about this item.

Dessert Items: in the item description box please specify dessert type, any possible allergen information (nuts, dairy, gluten), if the item needs to be refrigerated and if the item is a specific dietary friendly item (gluten free, dairy free, etc..)
